Your First
Visit
Welcome to our
practice. The following is what you can expect at
your initial appointment.
When you come in,
we'll ask you to fill out a patient questionnaire
and medical history. These forms are important--they
help determine the course of your treatment. We have
to know about allergies, sensitivity to anesthetics,
long-term medications--your whole health picture.
For your convenience these forms are available
below.
You can print,
complete and bring them to your first visit.
In addition, if you
have dental insurance we ask that you would provide
a copy of your card and filing information. We ask
that you update this information on an ongoing basis
once you've established yourself as a patient. The
information on file will by used unless otherwise
stated by the patient.
Then we'll need an
hour or so to examine your teeth, gums, jaw
alignment, and soft tissues. We'll take x-rays to
find areas of infection that need treatment, and, if
necessary, hard cast models will be made to examine
your bite. The exam includes a blood pressure check
and oral cancer analysis--again, to assess your
total health.

Fees & Insurance
Just
as we are committed to providing you with quality
dental care, we are concerned with keeping your
costs reasonable. One way we accomplish this is by
eliminating costly billing procedures and requesting
payment at the time of your visit. We accept Visa,
Mastercard and Discover.
Financing is also available through Care Credit. 3,
6, 12, 18 months interest free options are
available.
Dental Insurance:
Many companies in our area provide dental insurance
for their employees. If you are covered by dental
insurance, payment will be easy for you. We will
file your claim and, in most cases, accept
assignment of payment. You will only need to pay
your deductible and the percentage required by your
policy. We participate in Delta Dental of Iowa.
Payment Plans:
We are always willing to discuss payment plans with
you. Feel free to ask about a program to meet your
need for cosmetic or reconstructive surgery.
